Final Forms

All parents and guardians are encouraged to login to Final Forms and complete all forms for the 2025-2026 school year. Remember, Final Forms saves data from year-to-year meaning you will never need to enter the same information twice! It also pre-populates information whenever possible, for each of your students, saving you time. Student forms may be updated anytime throughout the school year with changes such as contact information, health history, medications, transportation, etc. Maintaining updated information is extremely important so that we may communicate with our REDBIRD families.

To start, be sure to select the 2025-2026 school year. It is also important to note that a parent or legal guardian must complete and sign all forms. Please carefully review all information, update if necessary, and sign all forms. Parents and guardians of incoming students in grades 7-12 must complete their information before their student can complete the process. Once you have completed your part, be sure to have your student login and complete the process.

A special note to parents of students attending the Ashland County-West Holmes Career Center: you and your student will need to complete all forms for our district as well as the Career Center. Final Forms does not share student information. It is extremely important that you complete all forms for both schools. We are sorry for any inconvenience this may cause.

Some helpful information-

Existing Account- New School Year

  • Login to your account with your username and password.

  • Once you login press the BIG BLUE button at the top and select 2025-2026

  • Be sure to complete and sign all forms.

Parent / New Student Registration

  • Go to https://loudonville-oh.finalforms.com

  • Click New Account under the parent icon 

  • Complete the fields on the registration page. Click - Register. You will receive an email within 3 minutes prompting you to confirm and complete your registration.

  • Login to your personal email and follow the link to confirm your account.

  • You will be prompted to set a password for your parent account.

  • Once you've set your password, you'll be taken to your home screen and you can now add your student(s).

  • Click the register student button.

If you have questions or require support completing Final Forms, contact the school office where your student attends or scroll to the bottom of the Final Forms page and click "Use Support."
